This undated Central Intelligence Agency record, filed under 1960, is a blank UFO Photographic Information Sheet meant to be completed by a person who photographed a UFO. It asks for camera details: the camera's name, manufacturer, lens data and focal length. It also asks for the date and time of the photographs, which way the photographer was facing, their location marked on a map, sketches of the surrounding area, and where and when the original negative was processed. It names no sightings.

Name of camera: Camera Manufacturer: Lense number and other data printed around lens: For the camera "bug"; what was the focal length of the lens-camera combination you were using? 2. Date pictures were taken: 3. Time of day pictures were taken (to the nearest minute, if you can): 4. Direction you were facing at the time of photography (North, South, East, West, etc.): 5. Your location at time of photography (indicate location on map, such as road map, and show direction you were facing when pictures were taken): 6.
