This U.S. Department of War file, date not given, reproduces badly scanned Army Air Forces check-list forms titled Unidentified Flying Objects, numbered as incident summaries 1 to 100 from the 1947 reports. Each form records the observer's name and occupation, place and time, the object's size, construction, direction, exhaust trails, weather and how it disappeared. Legible entries include a test pilot at Muroc AAF in July 1947, Portland City Police, Twin Falls, Hamilton Field, Harmon Field and a flight between Medford, Oregon and Mt. Shasta.

Each of these incident summaries includes a "Check-List - Unidentified Flying Objects" that contains details about the incident. Many summaries also include witness lists or statements and other narrative reports or descriptions.
