This U.S. Department of War file, undated, collects incident summary forms for a numbered series of unidentified aerial object reports, roughly incidents 173 to 233, largely from 1948. It includes reports such as a disc seen above clouds near Honolulu, an object near Sterling, Utah, a green fireball near North Powder, Oregon, and sightings near San Pablo and Pinole, California. Several entries carry evaluations, like one account judged extremely unreliable, and it closes with a blank guide to investigating unidentified aerial objects.

Each of these incident summaries includes a "Check-List - Unidentified Flying Objects" that contains details about the incident. Many summaries also include witness lists or statements and other narrative reports or descriptions.
