This is a New Zealand Defence Force public release of RNZAF file AIR 39/3/3, "Flying Saucers," which covers 1952 to 1955 and was declassified in September 2010 under the Official Information Act. It collects sighting reports from the public and military personnel, press clippings, and a table of 1952 sightings. It also holds Carter Observatory opinions attributing some reports to meteors. A large part covers the inquiry into lights seen from National Airways Corporation Flight 108 on 31 October 1955, which the RNZAF could not fully explain as Venus.

The following Questionnaire (Condensed Confidential Report For is forwarded to persons reporting strange Flying Objects. Where published reports do not print names of sighters the aid of that Newspaper's Editor is requested in readdressing C.S.I's enquiry to those concerned.

Personal and confidential report to Civilian Saucer Investigations from Person or Persons sighting strange or unidentified flying object or objects.

Date of sighting. What hour. Visibility. Locality of witness. Objects first sighted to NORTH/SOUTH/ EAST/WEST. (Delete as applicable). Direction of Flight. Sweep or length of Flight. Characteristics of Flight (dived, zigzagged, hovered etc.). How long observed. Angle above horizon. No. of objects. Shape of Flight Formation. Approx. height. Speed. Shape. Colour. Colour or light peculiarities. Sound. Trail (Vapour or exhaust). Did it appear solid, well-defined etc. Wind direction. Weather conditions at time. Nature of area (flat, hilly, etc.). Briefly explain how attention drawn to Flying Object. No. of Observers. Sex. Age. Witnesses' Names. Occupation. Special qualifications. Service experience (Army, Navy, Airforce etc.). General remarks. Is permission granted to the C.S.I. to publish names of witnesses in connection with this sighting in the Society's Bulletin "Quarterly Review". YES/NO
