This Federal Aviation Administration preliminary operations report is dated September 19, 2023. It says that at 5:45 PM EDT, Indianapolis Air Route Traffic Control Center relayed a report from Air Transport International flight 3443. The Boeing 767 was flying from LAL to RFD. Its crew described an unidentified aerial phenomenon, a metallic object, while northwest bound at flight level 400, about 21 miles west southwest of Louisville (SDF). No evasive action was taken and law enforcement was not notified.
