At 12:10 p.m. on 10/18/91, an individual identifying himself as telephonically contacted the Salt Lake City Division and spoke with the duty Agent, SA identified himself as an employee of

Utah, telephone number extension advised SA that while walking through the lobby of his place of employment on the morning of 10/18/91, he found the enclosed document lying on the lobby floor. stated that he read the document and due to the nature of the contents thereof, decided to contact the FBI. During the early afternoon of 10/18/91, delivered the enclosed document to the receptionist in the Salt Lake City Field Office.

Based upon the contents of the document and the manner in which it was recovered, Salt Lake City initially dismissed the document and its classification markings as a hoax. In an effort to determine possible DOD interest in the unusual document, via referenced sature talkall on 10/25/91, SSA was provided with a synopsis of the contents of the enclosed document, with a request for him to determine through his DOD contacts any DOD interest in "OPERATION MAJESTIC-12", U.S. AIR FORCE "PROJECT SIGN" or "PROJECT GRUDGE".
