This FAA release, obtained through The Black Vault, contains Air Transportation Security interview notes on FAA Form 1600-32. They cover the November 17, 1986 interview in Anchorage, Alaska, of the crew of JAL Flight 1628. Captain Kenju Terauchi and his crew reported yellow, amber and green lights ahead of and beside their Boeing 747, and the object also showed on the onboard radar. The notes add that Anchorage center and NORAD had radar targets, and that the only system problem was VHF static.
