This Central Intelligence Agency release, dated January 5, 1960, is a blank UFO Photographic Information Sheet for witnesses to fill in. It asks for the camera's name, manufacturer and lens data, including the focal length of the lens and camera combination. It also asks for the date, time and direction of the photographs, the photographer's location on a map, a sketch of the surrounding area, 360 degree ground orientation pictures, and where and when the original negative was processed.
